This code is for individual participation in an online program and should not be used for in-person or group sessions.

Medical necessity is established by the patient's risk factors for developing diabetes, such as prediabetes, family history, or obesity.

IMPORTANT:For in-person sessions, use 0403T. Do not report 0488T in conjunction with 98960, 98961, 98962, or 0403T.
