Removal of the battery component of the pulse generator for a wireless cardiac stimulator designed for left ventricular pacing.
Modifiers may apply based on specific payer guidelines and circumstances. Modifier 59 may be used when 0518T is performed as a distinct procedural service during the same session as another procedure. Other potential modifiers include but are not limited to 22 (increased procedural service), 78 (return to the operating room for a related procedure during the postoperative period), 79 (unrelated procedure or service by the same physician during the postoperative period) and should be utilized based on individual circumstances.
Medical necessity must be established for reimbursement. Accepted indications may include battery depletion, malfunction or erosion, device infection, patient intolerance, or upgrade to a new system. Supporting documentation, such as device interrogation reports, imaging studies, and clinical notes, should substantiate the need for the procedure.
When the patient is appropriately prepped and anesthetized, the provider makes an incision in the chest wall over the previously created subcutaneous pocket for the pulse generator battery of a wireless cardiac stimulator for left ventricular pacing. The provider removes the battery, disconnecting it from the cable connecting the battery to the system transmitter, if needed. The provider closes the incision.

The electrode component of the stimulator is typically not removed once implanted.
