2025 CPT code 11005

Debridement of skin, subcutaneous tissue, muscle and fascia for necrotizing soft tissue infection; abdominal wall, with or without fascial closure.

When reporting 11005, carefully consider the extent of the debridement. If other areas, such as the perineum or genitalia are also involved, a different code (11004, 11006) may be more appropriate. For debridement involving different depths, it might be necessary to sum the surface areas of the same depth and use different codes (11042-11047). Modifier 59 can be used if different depths are debrided in the same wound.

Modifiers may be applicable. Modifier 59 may be used to distinguish 11005 from codes 11042-11047 when different depths of tissue are debrided in the same operative session.

Medical necessity for 11005 is established by the presence of necrotizing soft tissue infection in the abdominal wall.Documentation should support the diagnosis and the extent of the infection.

The physician evaluates the extent of necrotic tissue in the abdominal wall.Using surgical instruments, they remove all infected and necrotic tissue, preserving as much healthy tissue as possible.Bleeding is controlled, drains may be placed, antibiotics are applied, and the wound is packed.Fascial closure may or may not be performed.

IMPORTANT:Related codes include 11004 (debridement of external genitalia and perineum) and 11006 (debridement of external genitalia, perineum, and abdominal wall). For debridement of other body areas or at different depths, see codes 11042-11047.Consider these alternatives depending on the specific location and extent of the infection.

In simple words: This procedure cleans a severe infection in the abdominal wall by removing all the dead and infected tissue, including skin, fat, muscle, and the tough layer covering the muscle. Sometimes, the surgeon will also close the covering over the muscle.

This code describes a surgical procedure involving the removal of dead, damaged, or infected tissue from the abdominal wall.It includes debridement of skin, subcutaneous tissue, muscle, and fascia. The procedure may or may not include closure of the abdominal fascia.

Example 1: A patient presents with a necrotizing soft tissue infection of the abdominal wall following a traumatic injury. Debridement of the skin, subcutaneous tissue, muscle, and fascia of the abdominal wall is performed (11005)., A patient with a post-surgical abdominal wound develops a deep infection involving the fascia and muscle. The surgeon performs debridement of the infected tissues of the abdominal wall (11005) along with fascial closure., A patient with diabetes develops a necrotizing infection of the abdominal wall. The physician performs debridement down to the muscle and fascia (11005) and leaves the wound open for later reconstruction.

Documentation should include the size and depth of the wound, the tissues involved (skin, subcutaneous tissue, muscle, fascia), the presence of necrotizing infection, any procedures performed (debridement, fascial closure), and the medical necessity for the procedure.
