2025 CPT code 11200

Removal of skin tags, multiple fibrocutaneous tags, any area; up to and including 15 lesions.

Refer to current CPT coding guidelines for proper use and reporting of 11200. Do not report 11200 and 11201 for the same group of lesions. If fewer than 10 additional lesions are removed after the first 15, use modifier 52 with 11201. Code 11200 is for sharp removal/excision. Simple ligation/tying off should be coded 17110/17111.

Modifiers such as 25 (Significant, Separately Identifiable E/M Service) and 59 (Distinct Procedural Service) may be applicable. Modifier 52 (Reduced Services) should be appended to 11201 if fewer than 10 additional lesions are removed.

Medical necessity is determined by the payer and may depend on factors such as size, location, symptoms caused by the skin tags (e.g., bleeding, pain, interference with clothing or other items), and the presence of inflammation or other complications.

The physician evaluates the skin tags, removes them using an appropriate method (scissors, blade, etc.), and applies an antibiotic. Local anesthesia may or may not be used.

IMPORTANT:For each additional 10 skin tag lesions removed, use add-on code +11201.For excision of benign lesions other than skin tags, see 11400-11446. For destruction of other skin lesions, see 17000-17286.

In simple words: Removal of up to 15 skin tags from anywhere on the body.

This code encompasses the removal of up to 15 skin tags from any area of the body using methods like scissoring, ligature strangulation, electrosurgical destruction, chemical destruction, or electrocauterization, with or without local anesthesia.

Example 1: A patient presents with 12 skin tags on their neck. The physician removes them using scissors after applying local anesthesia., A patient has 5 skin tags around their left eye. These are removed with electrocautery without anesthesia., A patient has 22 skin tags on their torso. The physician removes 15, billing 11200. The additional 7 would be billed separately using +11201-52 (reduced services).

Documentation should include the number and location of skin tags, the method of removal, and any anesthesia used. Medical necessity for removal (e.g., irritation, bleeding, patient request for cosmetic reasons) should also be documented.
