Implantation of a biologic implant (e.g., acellular dermal matrix) for soft tissue reinforcement (e.g., breast, trunk).This is an add-on code requiring a primary procedure code.

This is an add-on code, and should only be reported with a primary procedure code related to soft tissue surgery of the breast or trunk.Refer to the CPT manual for specific guidelines related to the primary procedure.

Modifiers may apply depending on the primary procedure and circumstances.Consult the CPT manual and NCCI edits for appropriate modifier usage. Modifier 50 should not be used with bilateral procedures.

Medical necessity is established by the need for soft tissue reinforcement to improve healing, prevent complications (e.g., hernia), or achieve optimal cosmetic results in the context of a primary surgical procedure. Documentation should clearly demonstrate the medical need for the implant and its role in improving the outcome of the primary procedure.

The surgeon prepares the biologic implant (hydration, shaping), positions it for tissue reinforcement, and attaches it using sutures.Wound closure and dressing are included.

IMPORTANT:For soft tissue reinforcement in areas other than breast and trunk, use code 17999. For topical application of skin substitute graft to a wound surface, see codes 15271-15278.

An acellular dermal matrix is implanted, and code 15777 is appended to the code for the primary wound repair procedure., A patient undergoes a bilateral breast augmentation.Acellular dermal matrices are used on both sides to reinforce the tissue. Code 15777 is reported twice, once for each breast.

Operative report detailing the type of biologic implant used, the location of implantation, the method of attachment, and the size and quantity of implant material used.Preoperative and postoperative photographs may also be beneficial for documentation.

** The supply of the biologic implant should be reported separately with appropriate HCPCS codes.This code should only be used when an acellular dermal matrix or similar biologic implant is used for soft tissue reinforcement.
