Code 19085 includes MR guidance, biopsy, placement of localization device (if performed), and imaging of the specimen (if performed). It should only be reported for the first lesion sampled under MR guidance during the same session. Additional lesions require use of add-on code +19086.

Modifiers may be applicable, such as 26 (professional component) or TC (technical component) if only part of the service was performed by the billing provider.

Medical necessity for this procedure must be supported by documentation of a suspicious breast lesion identified on imaging, inconclusive findings from other diagnostic tests, or other clinical indications that warrant tissue sampling for diagnosis.

In simple words: Using an MRI scan to guide the procedure, a small sample of tissue is removed from a suspicious area in the breast through a small needle. A tiny marker may be left at the biopsied site. The sample is then examined for abnormalities.

Code 19085 is reported for the first lesion, while add-on code 19086 is reported for the second lesion.
