Impression and custom preparation of an oral surgical splint.

This code should only be used when the physician personally designs and prepares the prosthesis. It should not be reported if the splint is prepared by an outside laboratory. It is not appropriate for impressions, TMJ orthotics, or morning repositioners. The date of service should be the delivery date of the splint.
