2025 CPT code 22859

Insertion of intervertebral biomechanical device(s) to disc space or vertebral body defect without interbody arthrodesis.

This code is reported per interspace or defect treated, not per number of devices inserted. It is for insertion without fusion. If fusion is performed, use 22853 or 22854. Report this code in addition to the primary procedure code.

In simple words: This procedure involves inserting a device like a cage or mesh into the space between vertebrae or over a bone defect in the spine to maintain spacing and height, without fusing the bones together. It is often part of a larger spine surgery.

Insertion of intervertebral biomechanical device(s) (e.g., synthetic cage, mesh, methylmethacrylate) to intervertebral disc space or vertebral body defect without interbody arthrodesis, each contiguous defect (List separately in addition to code for primary procedure).

Example 1: A patient undergoes a discectomy at L4-L5 and a biomechanical device is inserted to maintain disc height. 22859 is reported in addition to the discectomy code., After a corpectomy at T12, a biomechanical device is placed to cover the defect, without fusion. 22859 is reported with the corpectomy code. , Two cages are inserted into the L2-L3 disc space during a laminectomy. 22859 is reported once, along with the laminectomy code.

Documentation should specify the type of biomechanical device used, the location of insertion (interspace or defect), and the primary procedure performed.
