Injection procedure for shoulder arthrography or enhanced CT/MRI shoulder arthrography.

Follow CPT guidelines for arthrography and imaging procedures.Imaging is always reported separately from the injection procedure.

Modifiers 59 (distinct procedural service), 76 (repeat procedure), and others may apply depending on the circumstances and the associated imaging studies.

Shoulder arthrography is medically necessary when other imaging modalities (e.g., plain x-rays, ultrasound) are inconclusive and there is a clinical suspicion of intra-articular pathology (e.g., rotator cuff tear, labral tear, loose bodies, infection).

The physician or qualified healthcare professional is responsible for prepping the patient, inserting the needle under fluoroscopic guidance, injecting the contrast agent, and ensuring the joint is adequately filled. They are also responsible for ordering the subsequent imaging studies (x-ray, CT, or MRI).

IMPORTANT:For radiographic arthrography, radiological supervision and interpretation, use 73040. Fluoroscopy (77002) is inclusive of radiographic arthrography. When fluoroscopic guided injection is performed for enhanced CT arthrography, use 23350, 77002, and 73201 or 73202. When fluoroscopic guided injection is performed for enhanced MR arthrography, use 23350, 77002, and 73222 or 73223. For enhanced CT or enhanced MRI arthrography, use 77002 and either 73201, 73202, 73222, or 73223. To report biopsy of the shoulder and joint, see 29805-29826.
