Modifiers 50 (bilateral procedure), 51 (multiple procedures), 76 (repeat procedure), and possibly others depending on circumstances, may be applicable.

The procedure is considered medically necessary when conservative treatments (e.g., physical therapy) have failed to improve the patient’s condition.
The surgeon performs the incision, nerve release, tendon release, muscle release, transplantation of the ulnar nerve, and closure of the incision.This requires a deep understanding of hand and forearm anatomy and surgical technique.
