2025 CPT code 25607

Open treatment of distal radial extra-articular fracture or epiphyseal separation, with internal fixation.

Follow CPT guidelines for fracture and/or dislocation treatment. Use appropriate modifiers for additional procedures, bilateral procedures, or other circumstances that may impact coding.

Modifiers may be applicable depending on circumstances, such as bilateral procedures (modifier 50), assistant surgeon (modifier 80, 81, 82), or repeat procedures (modifier 76, 77).

Medical necessity for this procedure is established by documenting the presence of a fracture or epiphyseal separation requiring surgical intervention with internal fixation to ensure proper healing and restoration of function.

The surgeon opens the fracture site, reduces the fracture (puts the bone pieces back into alignment), and then applies internal fixation devices to hold the bones stable during healing. They also manage bleeding and close the surgical incision.

IMPORTANT (Do not report 25607 in conjunction with 25650) (For percutaneous treatment of ulnar styloid fracture, use 25651) (For open treatment of ulnar styloid fracture, use 25652)

In simple words: This procedure involves surgically repairing a broken bone in the lower part of your forearm, near your wrist, but not extending into the wrist joint. The surgeon makes an incision to access the broken bone and then uses metal hardware like pins, screws, or a rod inside the bone to hold the pieces together while they heal.

This code describes a surgical procedure to treat a fracture of the distal radius (the larger bone in the forearm) that does not involve the wrist joint (extra-articular) or an epiphyseal separation (fracture involving the growth plate) using internal fixation (such as pins, screws, wires, or an intramedullary nail).

Example 1: A 25-year-old male falls while skateboarding and sustains a distal radial extra-articular fracture. Code 25607 would be used if the fracture is treated surgically with internal fixation., A 10-year-old female falls from a tree and experiences a distal radial epiphyseal separation. Code 25607 would apply if the separation is treated surgically with internal fixation., A 60-year-old woman trips and falls, resulting in a distal radius fracture that does not extend into the wrist joint. Code 25607 would be appropriate if the fracture is treated surgically with open reduction and internal fixation using a plate and screws.

Documentation should include details of the fracture (e.g., location, type, displacement), the surgical approach used, the type of internal fixation devices employed, any complications encountered, and post-operative instructions.
