2025 CPT code 27066

Excision of deep bone cyst or benign tumor, wing of ilium, symphysis pubis, or greater trochanter of femur; includes autograft, when performed.

This code should not be reported in conjunction with radical resection codes for the same area. Separate codes exist for superficial lesions and for autografts requiring separate incisions. Careful consideration should be given to accurate measurement and documentation of lesion size and depth.

Modifiers may be applicable to indicate specific circumstances, such as increased procedural services (22), bilateral procedures (50), reduced services (52), or multiple procedures (51).

Medical necessity must be established by demonstrating the clinical symptoms and supporting imaging evidence of the bone cyst or benign tumor, along with the rationale for surgical intervention.

The physician is responsible for the complete surgical procedure, including preoperative planning, incision, dissection, tumor removal, bone grafting (if performed), wound closure, and postoperative care.

In simple words: This procedure removes a deep cyst or non-cancerous tumor from your hip bone. The surgeon makes a cut to reach the bone, removes the cyst or tumor, and may use a bone graft from your body to repair the area.

The procedure involves excising a deep (subfascial) bone cyst or benign tumor from specific areas of the pelvis and proximal femur, including the wing of the ilium, symphysis pubis, or greater trochanter. The procedure includes obtaining and applying an autograft, if necessary, to repair the bone defect created by the excision. The procedure also includes dissection and elevation of tissue planes required to access the tumor.

Example 1: A patient presents with a painful, deep-seated bone cyst in the wing of the ilium. The cyst is causing discomfort and limiting mobility. After imaging studies confirm the diagnosis, the physician performs CPT code 27066 to excise the cyst and repair the bone defect with an autograft., A benign tumor is discovered in the greater trochanter of the femur during a routine physical exam. The physician recommends surgical excision. The procedure is performed using CPT code 27066, with an autograft taken from the patient's iliac crest to fill the resulting bone cavity., A patient with a history of trauma experiences persistent pain in the symphysis pubis. Imaging reveals a benign bone tumor. The surgeon performs CPT code 27066, removing the tumor while preserving the surrounding healthy bone. An autograft is not required in this case.

Documentation should include the diagnosis, size and location of the lesion, operative report detailing the procedure, type of autograft used (if any), and any complications encountered.
