2025 CPT code 27253

Open treatment of a traumatic hip dislocation without internal fixation.

The application and removal of the first cast, splint, or traction device is included in the procedure. Separate reporting for subsequent replacement of these devices is allowed. This code is for open treatment without internal fixation. If internal fixation is performed, use the appropriate code.

Modifiers may be applicable. For example, modifier 54 may be used if another provider will be providing post-operative care.

Medical necessity for this procedure must be established by documenting the traumatic hip dislocation and failed attempts at closed reduction (if any), or the presence of contraindications to closed reduction.

The physician performs the surgical procedure to reduce the hip dislocation, including pre- and post-operative care. They are responsible for the diagnosis, treatment plan, and ensuring appropriate documentation.

IMPORTANT:For open treatment of a traumatic hip dislocation with acetabular wall and femoral head fracture, with or without internal or external fixation, use code 27254.

In simple words: The surgeon makes an incision to access the hip joint and put the hip bone back into its socket after it has been dislocated. No hardware like screws or pins are used to hold the bone in place.

This procedure involves surgically opening the hip joint to reduce a traumatic dislocation. The femoral head is relocated back into the acetabulum. This code does not include internal fixation.

Example 1: A 25-year-old male is involved in a motor vehicle accident and sustains a traumatic posterior hip dislocation. He is taken to the operating room for an open reduction of the dislocation without internal fixation., A 60-year-old female falls from a ladder and dislocates her hip. Due to osteoporotic bone, closed reduction attempts are unsuccessful, and she undergoes open reduction without internal fixation., A 30-year-old athlete suffers a traumatic anterior hip dislocation during a sporting event. Imaging reveals no associated fractures, and he undergoes open reduction without internal fixation.

Documentation should include details of the injury, physical exam findings, imaging studies confirming the dislocation, operative report detailing the open reduction procedure, and any associated injuries.
