Fasciotomy, foot and/or toe.
Modifiers may be applied depending on the circumstances of the procedure. For example, modifier 51 may be used if multiple procedures are performed during the same session. Modifier 76 may be appended if the procedure is repeated.
Medical necessity for a plantar fasciotomy is established when conservative treatments such as physical therapy, orthotics, and medications have failed to provide adequate pain relief or functional improvement. For toe contractures, medical necessity is established based on the severity of the contracture and its impact on mobility and daily activities.
The physician or qualified healthcare professional performs the fasciotomy, including incision, exposure of the fascia, release of the plantar fascia, rasping (if needed), irrigation, hemostasis (control of bleeding), and closure of the incision.Preoperative and postoperative care may be billed separately.
In simple words: The doctor makes a deep cut in the sole of the foot or toe to ease pressure or tightness caused by plantar fasciitis (painful foot inflammation) or a toe contracture. This involves releasing the tissue band on the bottom of the foot and may include smoothing the bone to reduce irritation.
A fasciotomy of the foot and/or toe involves a deep incision into the deep tissues of the sole of the foot or toe to relieve tension or pressure from plantar fasciitis (a painful inflammatory condition of the foot) or to release a contracture of the toe.The procedure includes exposing the deep fascia overlying the abductor hallucis and the medial plantar fascial band, detaching the plantar fascia from the heel bone, making incisions to release tension, possibly rasping the bony surface to reduce irritation, irrigating the area, checking for bleeding, and closing the incision.
Example 1: A patient presents with chronic plantar fasciitis unresponsive to conservative management.A plantar fasciotomy is performed to release the tension in the plantar fascia., A patient with a severe toe contracture limiting mobility undergoes a fasciotomy to release the contracted tissue and improve range of motion., A patient sustains a severe crush injury to the foot, resulting in compartment syndrome. A fasciotomy is performed to relieve pressure and prevent tissue damage.
Detailed history and physical examination documenting the diagnosis of plantar fasciitis or toe contracture (including duration, severity, and failed conservative treatments).Operative report detailing the surgical technique, including the location and extent of the incisions, tissue released, any rasping of the bone, and closure method.Postoperative progress notes documenting the patient's recovery and any complications.Imaging studies (if performed) showing the extent of the plantar fasciitis or contracture.
** The use of this code implies a deep fasciotomy. Superficial incisions and drainages are coded elsewhere.
