Radical plantar fasciectomy (separate procedure).

Medical necessity is established when conservative treatment options (e.g., physical therapy, medication, bracing) have failed to provide adequate relief of symptoms and the patient has persistent disabling pain and functional limitations due to plantar fasciitis or other conditions requiring radical excision. The surgeon performs the radical excision of the plantar fascia, including preparation, incision, dissection, excision of involved and uninvolved tissue, bone smoothing, potential abductor hallucis muscle fascia removal, and closure.

The plantar fascia is separated from the heel bone, and then the involved portion, along with additional uninvolved fascia (at least one-third), is excised. The bone surface is smoothed to promote healing. A thin piece of fascia from the abductor hallucis muscle may also be removed to prevent nerve entrapment. The incision is then closed.

Example 1: A patient with chronic plantar fasciitis unresponsive to conservative management undergoes a radical plantar fasciectomy to alleviate persistent heel pain and improve function., A patient with a recurrent plantar fibroma that has failed other treatment modalities undergoes a radical plantar fasciectomy to remove the tumor and surrounding tissue to prevent recurrence. , A patient with a severe contracture of the plantar fascia causing significant limitation of foot motion undergoes a radical plantar fasciectomy to release the contracture and restore normal foot function.
