Surgical removal of a bone spur from the heel bone (calcaneus), which may include a release of the plantar fascia.

The size of the spur is not relevant for coding purposes. If both a Haglund's deformity resection (28118) and a spur removal with or without plantar fascial release are performed, both codes can be billed.

Modifiers such as 50 (Bilateral Procedure), 76 (Repeat Procedure or Service by Same Physician), 77 (Repeat Procedure by Another Physician), and others may be applicable depending on the specific circumstances.

Medical necessity for 28119 must be established by documenting chronic heel pain due to a bone spur and plantar fasciitis, with failed conservative treatment attempts.

The physician applies a tourniquet to the ankle. An incision is made, and the plantar fascia is partially incised.
