2025 CPT code 29581

Application of a multilayer compression system to the leg (below the knee), including the ankle and foot.

Follow CPT guidelines for proper code selection and documentation.Appropriate modifiers (e.g., 50 for bilateral procedures, LT or RT for specifying the leg) should be used when necessary.Subsequent bandage changes are billed separately.

Modifiers 50 (bilateral procedure), LT (left side), and RT (right side) may be applicable.

Medical necessity for 29581 is established when the patient presents with a condition necessitating multilayer compression therapy, such as venous insufficiency, lymphedema, or post-surgical edema. The documentation must support the clinical indication for the procedure and demonstrate the medical necessity of the multilayer compression system.

The provider is responsible for ensuring the leg is clean and dry before applying the bandage. They apply multiple layers of bandages, assessing fit and pressure throughout the process. The provider may also be responsible for follow-up bandage changes (reported separately).

IMPORTANT:Do not use with 29540 or 29580 for the same extremity.Do not use with 36465, 36466, 36468, 36470, 36471, 36473, 36474, 36475, 36476, 36478, 36479, 36482, 36483 for the same extremity.Code 29582 is used for application to the thigh and leg.

In simple words: The doctor applies a special layered bandage to your lower leg, ankle, and foot. This helps improve blood flow and reduce swelling, particularly useful for people with leg problems related to poor blood circulation.

This CPT code describes the procedure of applying a multi-layer compression system to the lower leg, encompassing the area from below the knee to the ankle and foot. The application involves multiple layers of bandages to manage conditions such as venous insufficiency or lymphedema. This technique aims to improve blood flow, reduce swelling, and create a stable environment conducive to wound healing. The procedure includes assessment of fit and pressure during application.Subsequent bandage changes are reported separately.

Example 1: A patient presents with venous insufficiency and a venous stasis ulcer on their lower leg. The physician applies a multilayer compression system (29581) to improve blood flow, reduce edema, and promote wound healing., A patient with lymphedema in their lower extremity receives application of a multilayer compression system (29581) to reduce swelling and manage the condition. The physician documents the measurements of the leg and the pressure applied., Following a lower extremity surgical procedure, a multilayer compression system (29581) is applied to control post-operative edema and promote healing. The bandage is changed and reassessed regularly by the physician.

Detailed documentation should include the reason for the procedure (e.g., venous insufficiency, lymphedema, post-surgical edema), location of application (specifying right or left leg), type of bandages used, measurements of the leg prior to application, pressure applied, and any relevant clinical observations.Documentation for subsequent bandage changes should also be included if applicable.

** Accurate documentation is crucial for proper reimbursement.Ensure the documentation clearly supports the medical necessity for the procedure and aligns with payer specific guidelines.
