2025 CPT code 29826

Arthroscopy, shoulder, surgical; decompression of subacromial space with partial acromioplasty, with coracoacromial ligament (ie, arch) release, when performed.

Use 29826 in conjunction with 29806-29825, 29827, 29828. Do not report with open repair codes 23130, 23410, 23415, or 23412.

Modifiers such as 50 (bilateral procedure), 51 (multiple procedures), 59 (distinct procedural service), 76 (repeat procedure by the same physician), 77 (repeat procedure by another physician), LT (left side), and RT (right side) may be applicable based on the specific circumstances of the procedure.

Shoulder impingement causing significant pain and functional limitation despite conservative treatment. Confirmation of the diagnosis through physical examination and imaging studies. Documentation supporting the medical necessity of surgical intervention.

The surgeon examines the shoulder joint using an arthroscope, then inserts instruments to repair the shoulder blade and coracoacromial ligament. The procedure aims to alleviate shoulder impingement, which causes pain and irritation.The surgeon makes small incisions, inserts the arthroscope, and expands the joint area with saline solution. The arthroscope's camera allows visualization, and instruments are used to decompress the shoulder structures. Part of the coracoacromial ligament might be removed.Post-procedure, the area is irrigated, bleeding checked, instruments removed, and incisions closed.

Arthroscopic decompression of the subacromial space in the shoulder, including partial acromioplasty and release of the coracoacromial ligament. This is an add-on code and should be reported in addition to the primary procedure code.

Example 1: A patient presents with chronic shoulder pain and limited range of motion due to shoulder impingement. After conservative treatments fail, an arthroscopic subacromial decompression with partial acromioplasty and coracoacromial ligament release is performed in conjunction with a rotator cuff repair (29827)., A patient experiences persistent shoulder pain after a previous rotator cuff repair. Diagnostic arthroscopy reveals recurrent impingement. An arthroscopic subacromial decompression with partial acromioplasty and coracoacromial ligament release (29826) is performed in conjunction with revision rotator cuff repair., During an arthroscopic procedure to address a labral tear (29806), significant subacromial impingement is also observed. An arthroscopic subacromial decompression with partial acromioplasty and coracoacromial ligament release (29826) is performed concurrently.

Operative report detailing the arthroscopic findings, including the extent of subacromial impingement, the procedures performed (decompression, acromioplasty, coracoacromial ligament release), and any associated procedures. Preoperative imaging studies (e.g., MRI, X-ray) supporting the diagnosis of shoulder impingement. Documentation of failed conservative treatments prior to surgery.

** Code 29826 should only be reported when the coracoacromial ligament release is performed in conjunction with a subacromial decompression.
