2025 CPT code 33475

Replacement of the pulmonary valve.

Follow current CPT coding guidelines for cardiovascular surgery.

Modifiers may be used to indicate additional procedures performed during the same session (e.g., modifier 51 for multiple procedures) or to specify the type of anesthesia.

Medical necessity for pulmonary valve replacement is established by the presence of significant pulmonary regurgitation, right ventricular dysfunction, or symptoms attributable to pulmonary valve disease.Documentation should clearly link symptoms to the valve dysfunction.

The cardiac surgeon is responsible for all aspects of the procedure, from pre-operative planning and patient assessment to intraoperative technique and post-operative care.

IMPORTANT:May be reported with additional codes for other procedures performed during the same session, such as cardiopulmonary bypass (33369) or ventricular assist device placement (33990-33997), with appropriate modifiers.Related codes include those for repair of the pulmonary valve (33476, 33478).

In simple words: This surgery replaces the heart's pulmonary valve with an artificial one. This is sometimes needed for adults who had heart problems as babies, to fix a leaky valve that can cause the heart muscle to enlarge.

Surgical replacement of a diseased or malfunctioning pulmonary valve with a prosthetic valve. The procedure involves accessing the heart (typically via median sternotomy), opening the pulmonary artery, removing the native valve, and implanting a prosthetic valve of appropriate size. The incisions are then closed, and hemostasis is ensured.A bovine patch graft may be used to reinforce the closure if necessary.

Example 1: A 30-year-old patient with a history of Tetralogy of Fallot repair in infancy presents with pulmonary regurgitation and right ventricular hypertrophy.A surgical replacement of the pulmonary valve is performed., A 45-year-old patient with a bioprosthetic pulmonary valve that has degenerated requires valve replacement. The procedure is performed with cardiopulmonary bypass., A 25-year-old patient with congenital pulmonary valve stenosis undergoes pulmonary valvotomy; however, the valve is severely damaged, necessitating replacement. The patient also requires placement of a temporary ventricular assist device (VAD) due to low cardiac output. Both procedures are performed during a single surgical session.

Pre-operative echocardiogram demonstrating pulmonary valve dysfunction, operative report detailing the procedure and prosthesis used, post-operative echocardiogram demonstrating successful valve function. All pertinent lab results should be documented.

** This code represents only the surgical replacement of the pulmonary valve.Additional procedures such as cardiopulmonary bypass, coronary artery bypass grafting, or other cardiac interventions performed during the same surgical session should be reported with separate codes and appropriate modifiers.
