Medical necessity for this code requires documentation of a ruptured infrarenal aorta or iliac artery.
The physician performs the procedure under image guidance, including prepping, incision, graft deployment, checking for bleeding, and closure.
In simple words: This procedure involves repairing a ruptured area in the lower part of the aorta and/or iliac artery using a covered stent graft.A small incision is made in the groin, and the graft is inserted through a catheter and guided to the damaged area. The graft reinforces the artery wall and restores blood flow.
Endovascular repair of infrarenal aorta and/or iliac artery(ies) by deployment of an aorto-uni-iliac endograft including pre-procedure sizing and device selection, all nonselective catheterization(s), all associated radiological supervision and interpretation, all endograft extension(s) placed in the aorta from the level of the renal arteries to the iliac bifurcation, and all angioplasty/stenting performed from the level of the renal arteries to the iliac bifurcation; for rupture including temporary aortic and/or iliac balloon occlusion, when performed (e.g., for aneurysm, pseudoaneurysm, dissection, penetrating ulcer, traumatic disruption).
Example 1: A patient presents with a ruptured infrarenal aortic aneurysm. The physician performs an endovascular repair using an aorto-uni-iliac endograft (34704) to address the rupture., A patient with a ruptured iliac artery aneurysm undergoes endovascular repair with an aorto-uni-iliac endograft (34704). During the procedure, the physician uses temporary aortic and/or iliac balloon occlusion. , A patient experiences traumatic disruption of the infrarenal aorta. The physician performs an endovascular repair using an aorto-uni-iliac endograft (34704) to address the disruption, including angioplasty and stenting within the treatment zone.
Documentation should include details of the rupture, the type of endograft used, any extensions, associated procedures like angioplasty or stenting, and intraoperative findings.Pre- and post-procedure imaging reports are also required.
** Code 34704 is for ruptured aneurysms and traumatic disruptions. For non-ruptured conditions, other codes (e.g., 34703) should be used.
