2025 CPT code 34714

Open femoral artery exposure with creation of conduit for delivery of endovascular prosthesis or for establishment of cardiopulmonary bypass, by groin incision, unilateral. (List separately in addition to code for primary procedure)

Code 34714 is an add-on code and should be reported in addition to the primary procedure. It should not be reported if percutaneous access with a smaller sheath is feasible.

Modifiers may be applicable. Modifier 50 is used for bilateral procedures.

Medical necessity for 34714 is established when percutaneous access is not feasible or safe due to factors like large sheath size, patient anatomy, or other clinical considerations requiring open access.
