2025 CPT code 35152

Repair of a ruptured popliteal artery aneurysm via direct repair or graft with or without a patch graft.

Procedures 35001-35152 include preparation of artery for anastomosis including endarterectomy.

Modifiers such as 22 (Increased Procedural Services), 50 (Bilateral Procedure), 52 (Reduced Services), and others may be applicable depending on the specific circumstances of the procedure. Modifier -54 Surgical Care Only, -55 Postoperative Management Only, -56 Preoperative Management Only

Medical necessity is established by the presence of a ruptured popliteal artery aneurysm which poses an immediate threat to limb viability. Documentation should clearly illustrate the signs and symptoms necessitating surgical intervention.

The surgeon prepares the patient, makes the incision, exposes the popliteal artery, clamps the artery above and below the rupture, repairs the defect (either directly or with a graft), and closes the incision.

IMPORTANT:Use 35151 if the provider performs repair of an intact aneurysm in the popliteal artery.

In simple words: This procedure involves repairing a tear in the artery behind the knee. The surgeon makes an incision below the knee to access the damaged popliteal artery. The torn section is either stitched back together directly, or a replacement piece (graft) is used to bridge the gap. A patch may also be applied to reinforce the repair. This procedure is done when the aneurysm has already ruptured.

Direct repair of aneurysm, pseudoaneurysm, or excision (partial or total) and graft insertion, with or without patch graft; for ruptured aneurysm, popliteal artery.

Example 1: A patient presents with a pulsatile mass behind the knee, severe pain, and absent distal pulses, indicating a ruptured popliteal aneurysm. Code 35152 is used for the surgical repair., An athlete experiences sudden pain and swelling in the calf during strenuous exercise. Imaging reveals a ruptured popliteal artery aneurysm. Code 35152 is applied for the subsequent surgical intervention., A patient with a history of popliteal artery aneurysm develops acute limb ischemia. A ruptured aneurysm is identified as the cause. The surgical repair using a graft is coded as 35152.

Documentation should include operative report detailing the findings, the method of repair (direct or graft), use of patch graft, size of aneurysm, and any complications. Pre- and postoperative imaging reports should also be included.
