2025 CPT code 35682

Bypass graft; autogenous composite, two segments of veins from two locations. This code is reported in addition to the primary procedure.

Code 35682 is an add-on code and must be reported in addition to the primary bypass procedure code. It should not be reported with codes 35681 or 35683.

Modifiers may be applicable to indicate specific circumstances of the procedure (e.g., anatomical side, unusual complexity). Check current CPT guidelines for details on using modifiers 52,59 etc with this code.

Medical necessity is established by the underlying condition requiring bypass surgery, such as peripheral artery disease or coronary artery disease. The need for a composite graft should be justified due to inadequate or unsuitable autogenous vein at the typical harvest site.

The surgeon harvests two vein segments from separate locations, often a limb other than the one undergoing the bypass.The harvested veins are then joined together using fine sutures to create a composite graft, which is then used for the bypass procedure.

IMPORTANT:Use 35682 in conjunction with 35556, 35566, 35570, 35571, 35583-35587. Do not report 35682 in addition to 35681, 35683.
