Thrombectomy of arterial or venous graft (other than hemodialysis graft or fistula); with revision of arterial or venous graft.

This code should not be used for thrombectomy of hemodialysis grafts or fistulas.

Modifiers may be applicable. Refer to current CPT guidelines for modifier usage.

Medical necessity must be established by documenting the symptoms and diagnostic findings related to the clotted graft. The documentation should support the need for the intervention.

The surgeon is responsible for preparing the patient, making the incision, exposing the graft, controlling blood flow, removing the clot, revising the graft, restoring blood flow, and closing the incision.
