2025 CPT code 36002

Injection procedures (e.g., thrombin) for percutaneous treatment of extremity pseudoaneurysm.

Modifiers may be applicable to this code to indicate specific circumstances, such as increased procedural services (modifier 22), bilateral procedures (modifier 50), or distinct procedural services (modifier 59).Refer to current CPT coding guidelines for appropriate modifier usage.

Medical necessity for this procedure must be established by documenting the presence of the pseudoaneurysm, its size and location, and associated symptoms. The documentation should support the need for intervention to prevent complications such as rupture or thrombosis.

After the patient is prepped and the area is numbed, the physician uses ultrasound to find the pseudoaneurysm. A needle is inserted through the skin into the pseudoaneurysm, and thrombin is injected to promote clotting.

IMPORTANT:For imaging guidance, see 76942, 77002, 77012, 77021. For ultrasound-guided compression repair of pseudoaneurysms, use 76936. Do not report 36002 for vascular sealant of an arteriotomy site.

In simple words: The doctor injects a medicine into a false aneurysm in your arm or leg to help stop bleeding. This is done through the skin using a needle, and ultrasound is used to see where the needle needs to go.

This code describes a procedure where a clotting agent, such as thrombin, is injected into a pseudoaneurysm located in an extremity (arm or leg) using percutaneous access. The procedure typically involves ultrasound guidance to precisely locate the pseudoaneurysm sac and ensure accurate needle placement for thrombin injection.

Example 1: A patient presents with a pseudoaneurysm in the femoral artery following a cardiac catheterization. Code 36002 is used when the physician performs a percutaneous thrombin injection under ultrasound guidance., A patient develops a pseudoaneurysm in the brachial artery after an arterial blood gas draw. The physician uses ultrasound guidance to inject thrombin percutaneously into the pseudoaneurysm. This is coded as 36002., A patient with a history of trauma has a pseudoaneurysm in the radial artery. The physician performs a percutaneous thrombin injection using ultrasound guidance, which is reported with code 36002.

Documentation should include details of the pseudoaneurysm location, size, the method of injection (e.g., ultrasound-guided), the type of clotting agent used (e.g., thrombin), and the outcome of the procedure.
