Complete replacement of a peripherally inserted central venous catheter (PICC) without subcutaneous port or pump, through the same venous access, including imaging guidance, documentation, and radiological supervision and interpretation.

Use 37799 if the replacement is performed without imaging guidance.

Modifiers may apply based on specific circumstances (e.g. increased procedural services, reduced services).

Medical necessity for 36584 must be supported by documentation demonstrating the clinical need for PICC line replacement, such as infection, malfunction, or the need for a different type of catheter.

The physician is responsible for prepping and anesthetizing the patient, removing the old PICC line, inserting and guiding the new catheter into the correct position using imaging guidance, controlling bleeding, irrigating the insertion site, and ensuring proper documentation.
