2025 CPT code 37223

Revascularization of each additional ipsilateral iliac artery with transluminal stent placement, including angioplasty if performed. This is an add-on code used in conjunction with a primary procedure code.

* Code 37223 is an add-on code and must be used with a primary procedure code for iliac artery revascularization.* Report only one 37223 code for each additional ipsilateral iliac artery treated.* If angioplasty is performed within the same vessel as stent placement, it is included in the code.

Modifiers may be applicable depending on the circumstances of the procedure.Consult current CPT guidelines for modifier usage.

Medical necessity is established by the presence of significant hemodynamically significant stenosis or occlusion in the ipsilateral iliac artery causing symptoms such as claudication or rest pain, and the need for revascularization to alleviate these symptoms and improve blood flow to the affected limb.

The surgeon is responsible for pre-operative planning, including obtaining informed consent. The surgeon performs the procedure, which may include an incision, needle puncture, guidewire insertion, catheter placement, stent deployment, and angiography.Post-operative monitoring of vital signs and patient stability is also the surgeon's responsibility.

IMPORTANT:Use in conjunction with 37221 and 37222.Use with 34701-34711, 34718, 34845-34848 only when performed outside the endograft treatment zone.

In simple words: This code describes a procedure to improve blood flow in a blocked or narrowed artery in the leg or pelvis. It's only used when a doctor has already done a similar procedure on one artery on the same side of the body and needs to treat another artery on that same side. The doctor uses a small tube and a tiny mesh tube (stent) to open up the blocked artery.

This CPT code, 37223, represents an add-on procedure for the endovascular, open, or percutaneous revascularization of each additional ipsilateral iliac artery. The procedure involves transluminal stent placement to restore blood flow. Angioplasty within the same vessel is included if performed. This code is reported in addition to the code for the primary procedure and is only applicable when additional ipsilateral iliac vessels require revascularization after an initial vessel on the same side has already been treated.

Example 1: A patient presents with significant stenosis of both common iliac arteries. After successful angioplasty and stenting of the right common iliac artery (primary procedure), the physician proceeds to perform the same procedure on the left common iliac artery.Code 37223 would be used to bill for the left common iliac artery procedure., A patient undergoes an open surgical repair of an external iliac artery. During the procedure, another stenosis is identified in the internal iliac artery, on the same side. The physician utilizes an endovascular approach to place a stent. Code 37223 would be added to the primary procedure code., A patient with peripheral arterial disease (PAD) undergoes a percutaneous transluminal angioplasty (PTA) of the right common iliac artery.Subsequently, additional lesions are found in the right internal and external iliac arteries.Code 37223 would be reported for each additional vessel (internal and external iliac) treated.

* Pre-operative assessment and diagnosis of iliac artery stenosis.* Documentation of the initial procedure performed on the first ipsilateral iliac vessel.* Detailed operative report specifying the approach (endovascular, open, percutaneous), the number of additional ipsilateral vessels treated, and stent placement details.* Angiographic images demonstrating the stenosis and successful stent placement.* Post-operative assessment and documentation of patient's condition.

** This code is specifically for additional ipsilateral iliac vessels. For contralateral vessels or other vascular territories, different codes apply.
