The physician performs the procedure, including accessing the vein (open or percutaneous), advancing the catheter, performing angioplasty if needed, deploying the stent, and closing the access site.

Example 1: A patient presents with stenosis in multiple lower extremity veins.The physician performs a transcatheter stent placement in the femoral vein (37238) and then places an additional stent in the popliteal vein during the same procedure.37239 would be reported for the stent placement in the popliteal vein., A patient with deep vein thrombosis requires stent placement in the iliac vein and the femoral vein during the same operative session. Code 37238 would be reported for the placement of the first stent, and 37239 for the second stent., A patient undergoing dialysis has stenosis in multiple veins affecting the dialysis circuit. After a primary stent placement in one vein (37238), the physician decides to stent an additional vein during the same session to optimize dialysis access. Code 37239 would be reported for the stenting of the additional vein.
Documentation should include details of the vein(s) treated, the type of access (open or percutaneous), the method of stent placement, any associated angioplasty performed, and confirmation of stent placement.
** Catheter placement, ultrasound guidance, and diagnostic IVUS are not included in 37239 and may be reported separately when performed.
