Ligation of perforator veins, subfascial, radical (Linton type), including skin graft, when performed, open, 1 leg.

Code only once per leg. If bilateral, use modifier 50 or follow payer guidelines. Do not code graft harvesting separately.If ultrasound guidance is used for direct skin incision to access the perforator vein, use 37761 instead.

Modifiers 50 (bilateral procedure), 76 (repeat procedure), and potentially others depending on the circumstances of the procedure (e.g., 22 for increased procedural services, 52 for reduced services).

Medical necessity is established by symptomatic varicose veins causing pain, swelling, skin discoloration, or ulceration, unresponsive to conservative management.Documentation should include a thorough clinical examination and justification for the surgical intervention.
