Esophagogastroduodenoscopy (EGD) with directed submucosal injection(s) of any substance.
Modifiers 52 and 53 can be used if the duodenum is deliberately not examined during the procedure.

This code describes a procedure where a flexible endoscope is inserted through the mouth to examine the esophagus, stomach, and duodenum. During the procedure, one or more directed injections of any substance are made into the submucosal layer of these organs.This could include substances like botulinum toxin, steroids, India ink, or saline.
Example 1: A patient with achalasia undergoes EGD with injection of botulinum toxin into the lower esophageal sphincter to relax the muscle and improve swallowing., A patient with a submucosal lesion in the stomach undergoes EGD with injection of saline to lift the lesion, allowing for easier endoscopic resection during the same procedure., A patient with gastrointestinal bleeding undergoes EGD. During the procedure, a bleeding ulcer is identified in the duodenum and is injected with epinephrine to control the bleeding.
Documentation should include the indication for the procedure, the areas examined (esophagus, stomach, duodenum), the substance injected, the location of the injection(s), and any complications encountered.If the duodenum is not examined, the reason should be documented. Moderate sedation is not included and must be billed separately.
