2025 CPT code 43283

Laparoscopic esophageal lengthening performed in conjunction with a laparoscopic fundoplasty or paraesophageal hernia repair.

Code 43283 should not be reported separately if the esophageal lengthening is integral to the primary procedure. Refer to CPT guidelines and payer-specific policies for appropriate use and coding instructions.

In simple words: The surgeon uses a small camera and instruments inserted through tiny incisions to lengthen the esophagus. This procedure is done during the same surgery as a stomach wrap or hernia repair.

Laparoscopy, surgical, esophageal lengthening procedure (e.g., Collis gastroplasty or wedge gastroplasty) (List separately in addition to code for primary procedure). This add-on code describes a laparoscopic esophageal lengthening procedure performed during the same session as a separately reportable laparoscopic fundoplasty or paraesophageal hernia repair (43280, 43281, 43282).

Example 1: A patient with a paraesophageal hernia and shortened esophagus undergoes laparoscopic repair of the hernia (43282) along with a Collis gastroplasty to lengthen the esophagus (43283)., During a laparoscopic Nissen fundoplication (43280), the surgeon performs an esophageal wedge gastroplasty (43283) to address the patient's short esophagus caused by long-term GERD., A patient undergoes laparoscopic repair of a paraesophageal hernia without mesh implantation (43281) and also requires esophageal lengthening with a Collis gastroplasty (43283) due to esophageal stricture.

Operative report must confirm use of laparoscopic technique with description of esophageal lengthening including specific technique (e.g., Collis gastroplasty) and should indicate that it was done at the same time as laparoscopic fundoplasty or paraesophageal hernia repair. Document medical necessity for both the primary and add-on procedures.
