Percutaneous placement of ureteral stent and nephrostomy catheter; new access.

Medical necessity is established by a documented diagnosis of ureteral obstruction or other urologic condition that requires percutaneous nephrostomy and ureteral stent placement.The documentation should show that less invasive options have been considered and are not appropriate in this case.

The physician is responsible for pre-operative assessment,percutaneous access, catheter placement (both nephrostomy and ureteral stent), imaging guidance (ultrasound and/or fluoroscopy), radiological supervision and interpretation, and post-operative care.Anesthesiology services, if rendered, are billed separately.

IMPORTANT:For placement of only a ureteral stent through new access without a nephrostomy tube, use code 50694. For placement of a ureteral stent through a pre-existing nephrostomy tract, use code 50693.Do not report 50693, 50694, 50695 in conjunction with 50430, 50431, 50432, 50433, 50434, 50435, 50684, 74425 for the same renal collecting system and/or associated ureter.

This CPT code encompasses the percutaneous placement of both a ureteral stent and a nephrostomy catheter into a ureter and its associated renal pelvis during the same session, achieved through a newly created percutaneous renal access.The procedure includes access creation, drainage, catheter manipulations, diagnostic nephrostogram and/or ureterogram (if performed), imaging guidance (ultrasound and/or fluoroscopy), and all associated radiological supervision and interpretation.It does not include codes 50430-50435, 50684, or 74425 for the same renal collecting system and/or associated ureter.

Example 1: A patient presents with a ureteral stricture causing hydronephrosis. The physician performs a percutaneous nephrostomy and ureteral stent placement to relieve the obstruction and improve urine drainage., A patient with a kidney stone obstructing the ureter requires both nephrostomy tube placement for drainage and ureteral stent placement to facilitate stone passage.The physician performs the procedure using fluoroscopy guidance., A patient with recurrent urinary tract infections due to ureteral obstruction undergoes percutaneous placement of a ureteral stent and nephrostomy tube to decompress the kidney and allow for improved drainage, reducing the risk of infection.
