Circumcision using a clamp or other device with regional dorsal penile or ring block.

Code 54150 is used for circumcisions performed on patients of any age using a clamp or other device. For circumcisions performed using surgical excision (other than a clamp, device, or dorsal slit), use 54160 for neonates 28 days or younger and 54161 for patients older than 28 days.

Modifier 52 can be appended if the procedure is performed without the dorsal penile or ring block. Do not report modifier 63 in conjunction with 54150.

Medical necessity for circumcision may include routine neonatal circumcision for prophylactic reasons, phimosis (inability to retract the foreskin), paraphimosis (entrapment of a retracted foreskin), recurrent balanitis (inflammation of the glans penis), or posthitis (inflammation of the foreskin).
