Radical complete vulvectomy.

Modifiers may be applicable (e.g., 22 for increased procedural services, 51 for multiple procedures).

Medically necessary for malignant or pre-malignant conditions of the vulva that require complete resection to ensure optimal cancer control.

Performed by a gynecologic oncologist. The surgeon marks the excision area, makes an incision, and removes the vulvar structures.They control bleeding, may pack the vagina, and close the wound with sutures or a skin graft.

In simple words: This surgery removes all of the external female genital tissue, including the clitoris and labia, and sometimes parts of the upper thighs and lower abdomen. It's done to treat vulvar cancer. Sometimes a skin graft is needed to cover the area where the skin was removed.

This procedure involves the removal of the entire vulva, including the clitoris, labia majora and minora, and surrounding deep tissues. It may also include excision of portions of the upper thighs, lower abdomen, or perineum, along with the inguinal lymph nodes.The procedure aims to remove all cancerous tissue with a margin of healthy tissue. Reconstruction may involve layered suturing or skin grafts.

Example 1: A patient with invasive vulvar cancer involving the clitoris, both labia, and adjacent perineal skin undergoes a radical complete vulvectomy with bilateral inguinal lymph node dissection., A patient with multifocal vulvar carcinoma in situ (VIN) not amenable to less radical treatment undergoes a radical complete vulvectomy., A patient with recurrent vulvar cancer after previous local excision requires a radical complete vulvectomy.

Documentation should include the size and location of the tumor, depth of invasion, lymph node involvement (if any), type of reconstruction (sutures or skin graft), and operative report detailing the extent of resection.
