Medical necessity is established by the patient's history of pregnancy loss or risk factors for cervical insufficiency, as well as the potential for successful pregnancy outcomes following the procedure. This should be thoroughly documented in the records with clear justification for performing cerclage and the clinical benefits.
The physician is responsible for all aspects of the procedure, including patient positioning, anesthesia administration, surgical repair of the cervix, and post-operative care.
In simple words: This procedure repairs a weak cervix by stitching it stronger. It's done before pregnancy to help prevent miscarriage in women who have had miscarriages due to a weak cervix.It's a permanent fix, and future babies will need to be delivered by C-section.
This procedure involves the surgical reinforcement of the cervix, performed outside of pregnancy. The patient is placed in the lithotomy position, and a speculum is inserted into the vaginal canal. After general anesthesia, the cervix is exposed, and a uterine sound is used to locate the cervical defect. A transverse vaginal incision is made, the bladder is dissected away from the cervix, and the unhealthy cervical tissue at the defect is excised. The opening is closed in two layers with sutures, and a uterine sound is used again to ensure the vaginal canal remains open. The vaginal mucosa is then sutured back over the repaired area, and a vaginal pack may be inserted.
Example 1: A 30-year-old woman with a history of two miscarriages due to cervical insufficiency is scheduled for a nonobstetrical cerclage prior to attempting another pregnancy., A 25-year-old woman with a history of cervical trauma from a previous difficult delivery requires a cerclage to prevent future pregnancy complications., A woman with an anatomical abnormality of the cervix undergoes a nonobstetrical cerclage to reinforce the cervix and enable future childbearing.
Documentation should include the patient’s history of miscarriages or cervical trauma, the reason for the cerclage, the surgical technique used, and any complications encountered.
