In simple words: The surgeon removes the uterus, cervix, fallopian tubes, and ovaries through the vagina, using a laparoscope (a thin tube with a camera) to help detach the upper parts of the uterus and ovaries. This procedure is done when the uterus is of normal size (250 grams or less).

The procedure involves the removal of the uterus, cervix, fallopian tubes, and/or ovaries through the vagina, assisted by laparoscopy for the upper uterine and ovarian attachments.This code is specifically for a uterus weighing 250 grams or less.The procedure begins with laparoscopic dissection and detachment of the upper uterine and ovarian attachments, followed by vaginal removal of the uterus and other specified organs. The vaginal cuff is then closed vaginally.

Example 1: A 45-year-old woman with symptomatic uterine fibroids and a uterus weighing 200g undergoes LAVH with removal of both fallopian tubes and ovaries., A 50-year-old woman with abnormal uterine bleeding and a uterus weighing 180g undergoes LAVH with removal of both fallopian tubes, but preservation of the ovaries., A 55-year-old woman with early-stage endometrial cancer and a uterus weighing 220g undergoes LAVH with removal of the uterus, cervix, both fallopian tubes, and ovaries.

Operative report detailing the size of the uterus, the specific organs removed (uterus, cervix, fallopian tubes, ovaries), the laparoscopic and vaginal approaches used, and any intraoperative or postoperative complications. Preoperative imaging (ultrasound or MRI) to assess uterine size and any associated pathology.
