2025 CPT code 58900
Effective Date: N/A Surgery - Surgical Procedures on the Female Genital System Feed
Biopsy of ovary, unilateral or bilateral (separate procedure).
Modifiers may be applicable to this code, such as modifier 59 (Distinct Procedural Service) to indicate that the biopsy is a separate and distinct procedure from any other procedures performed at the same time.
Medical necessity for an ovarian biopsy must be supported by documentation of the patient’s symptoms, physical findings, or family history that suggest a need for the procedure to diagnose or rule out a specific condition.
The physician is responsible for performing the biopsy, obtaining the tissue sample, and ensuring proper handling and sending of the sample to the laboratory for examination.
In simple words: The doctor takes a small tissue sample from one or both of your ovaries to check for any problems. This can be done through your vagina or with a small cut in your lower belly. The sample is then sent to a lab for testing.
This procedure involves taking a tissue sample from one or both ovaries for diagnostic purposes. It can be performed through a vaginal or abdominal approach. In the vaginal approach, the patient is placed in the dorsal lithotomy position, a speculum is inserted into the vagina, and a small incision is made in the vaginal wall. The ovarian lesion is located, and a tissue sample is obtained using biopsy forceps. In the abdominal approach, the patient is placed in the supine position, and an incision is made in the lower abdomen. The abdominal cavity is examined, the ovarian lesion is located, and a tissue sample is taken. The tissue sample is sent to the laboratory for examination.
Example 1: A patient presents with an ovarian cyst. An ovarian biopsy is performed to determine if the cyst is benign or malignant., A patient has a family history of ovarian cancer. An ovarian biopsy is performed as a screening measure., A patient experiences abnormal vaginal bleeding. An ovarian biopsy is performed to rule out ovarian cancer.
Documentation should include the approach used (vaginal or abdominal), the size and location of the ovarian lesion, the method of obtaining the tissue sample, and any complications encountered during the procedure. The report from the laboratory analysis of the tissue sample should also be included.
** It is important to distinguish between a biopsy (58900) and a cystectomy (58925) which are distinct procedures with differing levels of complexity and reimbursement.
