2025 CPT code 64702

Neuroplasty of a digital nerve, involving one or both nerves in the same finger or toe.

Follow CPT coding guidelines for neuroplasty procedures. Ensure appropriate use of add-on code 64727 if internal neurolysis with an operating microscope is performed.

Modifiers may be applicable. Refer to current CPT guidelines and payer policies for correct modifier usage.

Medical necessity for 64702 must be supported by documentation of the patient's symptoms, physical examination findings, and failed conservative treatments. The documentation should demonstrate that the nerve compression significantly impacts the patient's function and quality of life and that surgical decompression is necessary to alleviate the symptoms.

The surgeon prepares the patient, administers anesthesia, makes an incision, identifies the compressed nerve, performs external neurolysis by dissecting scar tissue and other constricting elements, and closes the incision.
