Medical necessity is established when a patient experiences clinically significant symptoms (pain, numbness, paresthesia) due to compression of a plantar digital nerve that are not responding to conservative management.The procedure is considered medically necessary to improve nerve function and alleviate patient symptoms.

The surgeon is responsible for prepping the patient, making the incision, exposing and dissecting the constricting ligament, ensuring nerve decompression, and closing the incision.

IMPORTANT:Code 64727 should be added if internal neurolysis was performed using an operating microscope.Codes 62263, 62264, 62280-62282 are used for percutaneous neurolysis. Codes 69720 are used for facial nerve decompression. For neuroplasty with nerve wrapping, use 64702-64727, 64999.Do not report 64722, 64726 in conjunction with 11960.
