Nerve graft (includes obtaining graft), multiple strands (cable), hand or foot; up to 4 cm length

The harvesting of the nerve graft is included in 64895 and should not be reported separately. This code applies to grafts up to 4 cm in length; longer grafts are reported with 64896.

Modifiers may be applicable to further specify the circumstances of the procedure.Commonly used modifiers include 51 (Multiple Procedures) if performed with other procedures, or 59 (Distinct Procedural Service) under specific scenarios where the same surgeon performs distinct services on the same day.

Medical necessity for nerve grafting must be established by documenting the functional deficits caused by the nerve injury, and how the procedure is expected to improve the patient's condition.

The surgeon performs the entire procedure, including harvesting the nerve graft, preparing it, and implanting it to repair the damaged nerve. The sural nerve, superficial radial sensory nerve, or medial antebrachial cutaneous nerve are common donor sites.
