The physician performs the surgical repair of the nerve using a nerve allograft.
In simple words: The doctor repairs a damaged nerve using a piece of donated nerve tissue. This code is used for each additional nerve repair beyond the first one during the same procedure.
This procedure involves using a nerve tissue graft from a cadaveric or living donor to repair a nerve.The surgeon makes an incision, exposes the damaged nerve, and resects the damaged portion. An appropriately sized nerve allograft is placed between the nerve endings, and the nerve is repaired using microsurgical techniques. The code is used for each additional nerve strand repaired after the first.
Example 1: A patient sustains multiple nerve injuries in their hand requiring repair with nerve allografts. 64912 is reported for the first nerve repair and 64913 is reported for each additional nerve repaired., A patient with a traumatic leg injury requires repair of two separate nerve branches using allografts.64912 would be reported for the repair of the first nerve and 64913 for the second., During a complex surgical procedure, a surgeon repairs three damaged nerves using nerve allografts. 64912 would be reported once, along with two units of 64913.
Documentation should include the diagnosis, the specific nerves repaired, the type of graft used, the surgical technique employed, and any complications encountered. The operative report must clearly justify the medical necessity for repairing each nerve.
