Nerve repair with nerve allograft, each additional strand (List separately in addition to code for primary procedure).

Report 64913 for each additional nerve strand repaired with a nerve allograft beyond the initial repair (64912) during the same operative session.

Modifiers may be applicable to this code to indicate specific circumstances, such as the anatomical location of the repair or the presence of unusual complexity. Refer to current CPT guidelines.

Medical necessity for this procedure must be supported by documentation of functional deficits or other significant symptoms related to the damaged nerve(s).The documentation should also indicate why surgical repair with nerve allograft is the appropriate treatment.

The physician performs the surgical repair of the nerve using a nerve allograft.

IMPORTANT:Use 64913 in conjunction with 64912. Do not report 64912, 64913 in conjunction with 69990. For nerve repair with vein allograft, see code 64910, and for nerve repair with autogenous vein graft see code 64911.

Example 1: A patient sustains multiple nerve injuries in their hand requiring repair with nerve allografts. 64912 is reported for the first nerve repair and 64913 is reported for each additional nerve repaired., A patient with a traumatic leg injury requires repair of two separate nerve branches using allografts.64912 would be reported for the repair of the first nerve and 64913 for the second., During a complex surgical procedure, a surgeon repairs three damaged nerves using nerve allografts. 64912 would be reported once, along with two units of 64913.

Documentation should include the diagnosis, the specific nerves repaired, the type of graft used, the surgical technique employed, and any complications encountered. The operative report must clearly justify the medical necessity for repairing each nerve.
