2025 CPT code 65779

Placement of amniotic membrane on the ocular surface; single layer, sutured.

Refer to CPT guidelines for ophthalmologic procedures.Modifiers 50 (bilateral procedure), RT (right eye), or LT (left eye) may be used as appropriate.

Modifiers 50, RT, and LT are applicable.

Medical necessity for 65779 is established by the presence of a corneal or conjunctival defect that is not responding to conservative management and requires surgical intervention for healing and/or prevention of vision loss.Documentation must support the severity of the condition and the need for amniotic membrane transplantation.

Preparation and draping of the eye, removal of diseased epithelium, placement and suturing of the amniotic membrane.

IMPORTANT:Do not report 65778, 65779 in conjunction with 65430, 65435, 65780. For placement of amniotic membrane using tissue glue, use 66999. Do not report 65760-65771 in conjunction with 92025.

In simple words: A thin layer of tissue from a placenta is placed on the surface of the eye to help heal a wound or injury.It's held in place with stitches.This may need to be done again later.

The provider prepares and drapes the eye in a sterile manner.The diseased or necrotic epithelium is removed. A single layer of amniotic membrane is trimmed and applied to the ocular surface, covering the lesion. The membrane is secured with interrupted sutures to the cornea.Growth factors within the amniotic membrane stimulate healing, tissue repair, and inhibit unwanted vessel formation.The effects are temporary, and repeat procedures may be necessary.

Pre-operative assessment of the patient's ocular condition, including photographs and imaging if necessary. Intra-operative documentation of the procedure, including type and amount of amniotic membrane used, suture technique, and post-operative care instructions.Post-operative examination notes documenting healing progress.
