2025 CPT code 67801

Excision of multiple chalazia from the same eyelid.

Refer to the current CPT coding guidelines for detailed instructions and updates.

Modifiers may be applicable depending on the circumstances of the procedure. For example, modifier 51 (multiple procedures) may be used if other procedures are performed on the same day.

Surgical excision of chalazia is medically necessary when conservative management (warm compresses, topical medications) fails to resolve the condition, particularly when the chalazia cause significant visual impairment, persistent inflammation, or cosmetic concerns.Medical necessity may also be established when there is concern for malignancy.

The ophthalmologist or qualified surgeon is responsible for the entire procedure, including pre-operative assessment, anesthesia administration (if applicable), surgical excision of the chalazia, post-operative care, and follow-up.

IMPORTANT:67800 (single chalazion excision), 67805 (multiple chalazia on different lids), 67808 (chalazion excision under general anesthesia)

In simple words: This surgery removes multiple small bumps (chalazia) from one eyelid. The doctor will numb the area, make tiny cuts to remove the bumps, and then clean and treat the area to prevent infection.

This procedure involves the surgical removal of multiple chalazia (small, benign cysts) located on the same eyelid. The provider will typically administer local anesthesia, then use forceps to expose the chalazia.A small incision is made into each chalazion to allow for the removal of the contents and any associated fibrous tissue. The incision site is then irrigated with sterile saline, hemostasis is achieved, and antibiotic ointment is applied.

Example 1: A 45-year-old patient presents with multiple chalazia on their right upper eyelid, causing significant cosmetic concern and mild irritation. The physician performs an excision of the multiple chalazia on the same eyelid using local anesthesia., A 60-year-old patient with a history of blepharitis presents with recurrent chalazia on their left lower eyelid. The physician removes multiple chalazia from the same lid under local anesthesia., A 28-year-old patient experiences multiple recurring chalazia on their upper left eyelid, causing significant discomfort and visual impairment. The physician opts for surgical excision of multiple chalazia from the same eyelid, utilizing local anesthesia.

* Thorough history and physical examination documenting the presence and location of multiple chalazia on the same eyelid.* Preoperative photographs of the affected eyelid.* Operative report detailing the procedure, including the number of chalazia excised, anesthesia used, and any complications.* Postoperative assessment, including any complications.
