Modifiers 26 (professional component) and TC (technical component) may be appended as appropriate based on the service rendered and payer policy.Modifier 52 (reduced services) can be used if only parts of the exam are completed.

Medical necessity for a barium swallow is established when there's clinical suspicion of esophageal disorders that impact swallowing, such as dysphagia, GERD, or post-surgical evaluation.

The radiologist positions the patient, administers the contrast (barium), acquires the images using fluoroscopy and/or static X-rays, interprets the images, and prepares a detailed report of the findings.

IMPORTANT:Do not report 74220 in conjunction with 74221, 74240, 74246, or 74248.For a double-contrast study with high-density barium and an effervescent agent, use 74221.
