2025 CPT code 74485

Radiological supervision and interpretation for dilation of ureter(s) or urethra.

For services paid under OPPS, Medicare does not pay hospitals separately for supervision and interpretation codes considered ancillary to primary services. However, hospitals should still report the code for statistical purposes.

Modifiers 26 (Professional Component) and TC (Technical Component) can be used if reporting only one component of the service. Payer policies may vary regarding modifier TC for hospitals.

Medical necessity for 74485 is established by the presence of a ureteral or urethral stricture causing clinically significant symptoms or complications.

IMPORTANT:Do not report 74485 in conjunction with 50436, 50437. For dilation of ureter without radiologic guidance, use 52341, 52344. For change of nephrostomy or pyelostomy tube, use 50435. For dilation of a nephrostomy tract for endourologic procedure, see 50436, 50437.

In simple words: The doctor uses X-ray imaging to guide and interpret the widening of the tubes that carry urine from the kidneys to the bladder (ureters) or the tube that carries urine from the bladder out of the body (urethra). This code covers the doctor's expertise in reading the images and overseeing the procedure.

The provider performs imaging supervision and interpretation for the dilation (widening) of the ureters or the urethra. This code includes both the technical and professional components. It is used for imaging supervision and interpretation only. The procedure involves prepping and anesthetizing the patient, then under fluoroscopic guidance, dilating the ureters or urethra. Dilators of increasing sizes are inserted over a guidewire into the narrowed structure. A balloon catheter is passed over the guidewire to the stricture, and the balloon is inflated to dilate it.

Example 1: A patient presents with a ureteral stricture causing hydronephrosis. The urologist performs a ureteral dilation under fluoroscopic guidance, and the radiologist supervises and interprets the imaging., A patient with a urethral stricture undergoes a dilation procedure.A radiologist provides and interprets the fluoroscopic imaging for the procedure. , Following prostate surgery, a patient develops a urethral stricture. The urologist dilates the urethra under fluoroscopic guidance provided and interpreted by a radiologist.

Documentation should include details of the stricture, the dilation procedure, the use of fluoroscopy, and the interpretation of the images.Medical necessity for the procedure must also be documented.
