Computed tomographic angiography of the heart, coronary arteries, and bypass grafts (if present), using contrast, with 3D image post-processing.

Only one computed tomography heart service should be reported per encounter. For separate injection procedures for vascular radiology, see Surgery section, 36000-36299. For cardiac catheterization procedures, see 93451-93572.

Modifiers such as 26 (professional component), TC (technical component), and 52 (reduced services) may be applicable depending on the specific circumstances of the service provided.

Example 1: A patient with a history of chest pain and abnormal EKG undergoes a CCTA to evaluate for coronary artery disease., A patient with a known coronary artery bypass graft undergoes CCTA to assess graft patency and cardiac function., A patient with suspected congenital heart disease undergoes a CCTA to visualize the heart's anatomy and blood flow.

Documentation should include the medical necessity for the procedure, details of the contrast administration, and the physician's interpretation of the 3D images. It must also contain information about bypass grafts present, if performed.

** Code 75574 includes the evaluation of cardiac structure and morphology, assessment of cardiac function, and evaluation of venous structures when performed. Do not report noninvasive estimate of coronary fractional flow reserve derived from augmentative software analysis with this code, use 75580 instead.
