2025 CPT code 76776

Ultrasound examination of a transplanted kidney using real-time and duplex Doppler imaging with image documentation.

If a limited number of organs or a limited portion of the region is evaluated, use the "limited" code (76775) instead of the "complete" code. Do not report a limited exam of the same region in the same session as a complete exam of that region.

Modifiers 26 (Professional Component) and TC (Technical Component) may be applicable depending on who performs and interprets the study.

Medical necessity must be established by the patient's clinical presentation, such as signs or symptoms of transplant complications, or as part of routine post-transplant surveillance.

IMPORTANT:Do not report 76776 in conjunction with 93975 or 93976 when performing ultrasound and duplex Doppler of a transplanted kidney. For ultrasound of a transplanted kidney without duplex Doppler, use 76775.

In simple words: This ultrasound exam uses sound waves to create pictures of your transplanted kidney and check the blood flow to ensure it's working properly.

This code represents a comprehensive ultrasound evaluation of a transplanted kidney, utilizing both real-time imaging to visualize the kidney's structure and duplex Doppler to assess blood flow within the organ and its associated vessels. The procedure includes a final written report and image documentation. It's crucial for monitoring the health and function of the transplanted kidney, evaluating vascular patency, and detecting potential complications.

Example 1: A patient with a recent kidney transplant presents for a follow-up evaluation. Code 76776 is used to assess the kidney's blood flow and overall function., A patient with a transplanted kidney experiences decreased urine output. Code 76776 is used to evaluate for vascular complications, such as stenosis or thrombosis., A patient with a history of kidney transplant presents with abdominal pain. Code 76776 is used to rule out rejection or other post-transplant complications.

Documentation should include real-time images with measurements where clinically indicated, a final written report detailing the evaluation, a description of the visualized elements or reasons for non-visualization, and documentation of the duplex Doppler assessment of blood flow.
