Transabdominal ultrasound examination of each additional fetus after the first trimester (14 weeks or later).

Code 76810 is an add-on code and is always reported in addition to 76805 for the first fetus. Modifier 26 should be used when reporting only the professional component, and TC should be used when reporting only the technical component (though payer rules may vary).

Modifiers may apply. Modifier 59 is used to distinguish each additional fetus after the first, modifier 26 for the professional component and TC for the technical component.

Medical necessity for this code is established by the existence of a multiple gestation pregnancy beyond 14 weeks 0 days, requiring assessment of the additional fetuses.

The physician or other qualified healthcare professional performing the ultrasound is responsible for applying the ultrasound gel, positioning the transducer, obtaining the necessary images, interpreting the ultrasound findings, and documenting the results in the patient's medical record.

IMPORTANT:Use 76810 in conjunction with 76805 (for the first fetus). For a limited or follow-up ultrasound assessment, consider 76815 or 76816. For non-obstetrical transvaginal examinations, use 76830.

When visible, examination of maternal adnexa is also included.

Example 1: A pregnant woman at 15 weeks gestation is confirmed to be carrying twins. Code 76805 is used for the ultrasound of the first fetus, and 76810 is used for the ultrasound of the second fetus., A pregnant woman at 20 weeks gestation is expecting triplets. An ultrasound is performed, with 76805 reported for the first fetus and 76810 reported for each of the two additional fetuses., A patient presents at 16 weeks gestation for a routine ultrasound. A previous ultrasound indicated twins. The ultrasound confirms twins. 76805 is reported for the examination of the first fetus, and 76810 is reported for the second fetus.

Documentation should include the number of fetuses, gestational age, measurements of each fetus, evaluation of fetal anatomy (including intracranial/spinal/abdominal structures, heart, umbilical cord insertion site, placenta location), assessment of amniotic fluid, and examination of maternal adnexa when visible. The report should also document the reason for any non-visualization of structures.
