2025 CPT code 76872

Transrectal ultrasound examination of prostate.

All diagnostic ultrasound examinations require permanently recorded images with measurements, when such measurements are clinically indicated. For those codes whose sole diagnostic goal is a biometric measure (ie, 76514, 76516, and 76519), permanently recorded images are not required. A final, written report should be issued for inclusion in the patient's medical record.

Modifiers 26 (Professional Component) and TC (Technical Component) may be applicable.

Medical necessity must be established for each transrectal ultrasound. This might include abnormal digital rectal exam findings, elevated PSA, or other relevant clinical indicators.

In simple words: The doctor uses a small ultrasound probe inserted into the rectum to create images of your prostate. This helps them check the size and look for any problems.

This code represents a transrectal ultrasound examination of the prostate. It involves inserting an ultrasound probe into the rectum to obtain images of the prostate gland. This procedure is commonly used to assess prostate size, identify abnormalities, and guide biopsies.

Example 1: A 60-year-old male presents with elevated PSA levels. A transrectal ultrasound (76872) is performed to evaluate the prostate for abnormalities and guide a potential biopsy., A patient is undergoing prostate brachytherapy treatment planning. A transrectal ultrasound (76872) is performed to determine prostate volume for accurate dosimetry calculations., A patient with a history of prostatitis experiences recurrent symptoms. A transrectal ultrasound (76872) is performed to assess the prostate for inflammation or abscess.

Documentation should include the indication for the exam, a description of the findings, including measurements when clinically indicated, and a final, written report.
