2025 CPT code 79200

Radiopharmaceutical therapy, by intracavitary administration.

This code does not include the supply of the radiopharmaceutical, which should be reported separately. If imaging guidance is used, it should also be coded separately. Modifiers 26 (professional component) or TC (technical component) should be appended as appropriate if only one component of the service is being reported.

Modifiers 26 and TC can be used to report only the professional or technical component of the service, respectively.

Medical necessity must be established by demonstrating the clinical appropriateness of intracavitary radiopharmaceutical therapy for the patient's specific condition. Supporting documentation, such as imaging studies and pathology reports, should be provided.

The physician performing the procedure is responsible for selecting the appropriate radiopharmaceutical, calculating the correct dosage, administering the substance safely into the body cavity using an applicator, and monitoring the patient during and after the procedure. They must also ensure proper radiation safety protocols are followed.

In simple words: This procedure involves placing a radioactive material inside a body cavity, like the uterus, to treat conditions such as cancer. The radioactive material is placed close to the tumor to target the cancerous cells while minimizing damage to healthy tissues.

This code represents the administration of a radiopharmaceutical substance directly into a body cavity, such as the uterus, for therapeutic purposes. This method is often used in nuclear medicine for targeted treatment of certain conditions like cancer, where the radioactive substance is placed near the tumor within the cavity.

Example 1: A patient with endometrial cancer undergoes intracavitary brachytherapy with a radiopharmaceutical agent administered directly into the uterine cavity., A patient with a malignant pleural effusion receives intracavitary radiopharmaceutical therapy into the pleural space., A patient with peritoneal carcinomatosis undergoes intraperitoneal administration of a radiopharmaceutical agent for targeted therapy.

Documentation should include the type and dose of radiopharmaceutical used, the specific cavity into which it was administered, the method of administration, the duration of the procedure, and any associated imaging guidance used. Any pre- and post-procedure instructions given to the patient should also be documented.
